Wellbeing Services

Country Connect Foundation provides comprehensive wellbeing services for participants funded by the NDIS and various other sources. Our mission is to transform lives by providing equitable access to support services for all. To that end, we are committed to delivering essential support to individuals who may be ineligible for or do not meet the criteria for NDIS funding by leveraging alternative funding streams.

Our Wellbeing Services

  • These services assist participants with daily activities, ensuring they can live independently and engage fully in their communities. Our team is here to provide the necessary support for personal care, meal preparation, transportation, and more.

  • We focus on helping participants develop the skills they need to lead fulfilling lives. This may include training, skill development, and resources aimed at enhancing personal independence and confidence.

  • We provide General Wellbeing Services in various remote and regional areas including;

    • Greater Darwin

    • Tiwi Islands

    • Jabiru

    • Ramingining

    • Gapuwiyak

    • Galiwin’ku

    • Nhulunbuy

  • Our General Wellbeing Services are designed for individuals of all backgrounds who are seeking support to enhance their overall health, social connections, and daily living experience. These services are ideal for participants who benefit from additional assistance in areas like mental health support, social engagement, physical wellness, and cultural reconnection.

    This program is particularly suitable for individuals who:

    • Are funded through the NDIS or other alternative funding sources

    • Are seeking a holistic approach to wellbeing that addresses physical, emotional, social, and cultural needs

    • May need support to overcome barriers related to accessing community, healthcare, and recreational activities, especially in remote or regional areas

    • Are interested in engaging in meaningful experiences, and connections within positive communities

    Our wellbeing services are adaptable to each participant's unique goals and circumstances, ensuring personalised, culturally aware care that empowers individuals to lead fulfilling, connected lives.

  • We are dedicated to building a team that reflects the lived experiences, cultural backgrounds, and language diversity of the communities we serve. By prioritising these shared connections, we create an environment where participants feel understood and supported in a way that respects their cultural needs. This two-way approach not only promotes participants' wellbeing through a foundation of trust and familiarity but also upholds mainstream regulatory standards, ensuring balanced, inclusive care. Thanks to donor contributions, individuals living with disability, including those not covered by standard government funding, can access essential services and resources, creating opportunities for all to achieve their fullest potential.
